The formula for HOMA-IR is: (Fasting Insulin [µIU/mL] × Fasting Glucose [mmol/L]) / 22.5
To convert glucose from mg/dL to mmol/L, divide by 18.
Optimal target: < 1.0. Early insulin resistance: > 1.9. Significant insulin resistance: > 2.9.
About HOMA-IR
The Homeostatic Model Assessment of Insulin Resistance (HOMA-IR) is a method used to quantify insulin resistance and beta-cell function from basal (fasting) glucose and insulin or C-peptide concentrations.
While a fasting glucose test only tells you the amount of glucose in your blood at that moment, it doesn't tell you how hard your pancreas is working (via insulin secretion) to keep that glucose number normal. HOMA-IR provides a clearer picture of metabolic health years before HbA1c or fasting glucose levels rise to pre-diabetic thresholds.
